Job Description:
Position Overview
The ID/DD Coordinator is responsible and accountable for the
coordination and administration of services for clients and their
caregivers under the direct supervision of the Location or
Executive Director.
Essential Job Functions
--- Provide initial evaluation to determine appropriateness and
safety of client and environment.
--- Follow policies and procedures and conforms to all applicable
patient care standards, licensure regulations and state laws
--- Provide a written summary for individual clients and services
based on needs, goals, and expected outcomes in conjunction with
the ISP to Supports Coordinators/Administrative Entities on a
monthly basis
--- Participate in the development of Individual Support Plan
--- Maintain patient confidentiality
--- Demonstrate an understanding of client's medical status and
updates self on clients/family status as needed or requested
--- Maintain effective communication between staff, supports
coordinators, and family
--- Review progress notes from paraprofessionals to determine
appropriate service delivery
--- Educate paraprofessionals on individual client ISP
expectations
--- Perform on site evaluation of employee performance at 90 days
and annually
Requirements
--- Bachelor's degree in Psychology or other social science or
related field
--- Current CPR certification
--- Proof of eligibility to work in the United States
--- Current completion of child abuse clearance and
Fingerprinting

--- Minimum of one year of experience with ID/DD preferred

Physical Requirements
--- Must be able to speak, write, read and understand English
--- Must be able to travel; company does not provide vehicles or
transportation
--- Frequent lifting, carrying, pushing and pulling of 25
pounds
--- Occasional lifting, carrying, pushing and pulling of 50
pounds
--- Prolonged walking, standing, bending, kneeling, reaching,
twisting
--- Must be able to sit and climb stairs
--- Must have visual and hearing acuity
--- Must have strong sense of smell and touch
Environment
--- Must be able to function in a wide variety of environments
which may involve exposure to allergens and other various
conditions
--- Possible exposure to blood, bodily fluids and infectious
diseases
